Suppressive therapy throughout the pregnancy is warranted for persistent ABU. Early empirical treatment in acute cystitis awaiting culture results -better outcome. Acute pyelonephritis poses risk of maternal and fetal complication in more than 20% of cases. Early parenteral broad-spectrum antibiotics are warranted, can be switched to oral tailored to susceptibility profile once patient is afebrile. Penicillins, cephalosporins, aztreonam and fosfomycin are the safest alternatives. Nitrofurantoin and TMP-SMX to be avoided in 1st trimester and at term.
